Services

Consultation:

Consulting Services: While most of my time is spent one on one in therapy, I also do a significant amount of “consultation” with persons of diverse professional and personal backgrounds. An example of a “Consultation” might be a business owner, and professionals (i.e. physician or attorney) who is planning or needing to make changes and wants clarity/guidance about those organizational, personnel, personal changes, or someone needing to explore and gain insights into cultural, familial, or organizational dynamics to help make decisions, either personally or in their business.

Limitations to services:
If it becomes apparent at any time throughout the consultation or therapeutic process that my outpatient services may not be sufficient/adequate or are outside my scope of practice, this will be discussed, a referral or recommendation for treatment or other providers may be offered and counseling terminated. Be aware that I do not do any form of forensic or court related service or evaluations or employment related evaluations. It is unlikely that I can write a letter for you to the court or probation officer. More detail about my scope of service can be seen in the policies and consents documents.

How to Inquire about Services or Make an Appointment:

Please note that I do a free brief phone consultation before setting an initial consultation-interview appointment in order to help make sure that my practice is a good fit for each client I accept. There may be times when the best service I can provide is to try to make a good referral.

  1. Please review my website for basic information about my practice, my fee schedule, and limits to confidentiality and privacy, etc.
  2. If you agree to my practice policies or have questions call my work phone, 940-395-1670. If I don’t pick up, please leave a short message, your name and the number you want me to use to return your call. Also if timing of my return call is a concern, i.e. privacy, please give me several time blocks in which to call back when you can answer the phone. If you were referred by another professional, please also leave their name and number as well. Note that I often cannot answer the phone during the day due to my schedule, but will return your call as soon as I am able, usually by 6:30 pm the same day. .
  3. I will call to schedule a time for a brief 15 minute phone consultation with whoever the client is. This consultation will help me know whether the scope of my practice and expertise is appropriate for your needs. If not, I’ll be glad to try to give you a recommendation.
  4. The first (in-person or by teletherapy) appointment, as a consultation/interview, will usually be for 55-75 minutes. Before the consultation, you will need to go to my Forms Page and review all of the essential documents, such as Notice of Privacy Practices, Consent for Treatment information detailing the specific information about risks and benefits of therapy and my policies and procedures, then print, complete and bring the history questionnaire to the first interview. After the initial consultation, I will make a recommendation for treatment. If accepted as a client, a next session may be scheduled, (typically 45-50 minutes), and a therapeutic agreement will be signed. Until that time, I am acting as a consultant and trying to answer your questions and assess your situation and needs. If we do not work together, I’ll do what I can to make a helpful referral/recommendation.
Please Note: Limitations of Services

Privacy of communication forms: I strive to meet all “best practices”, HIPAA/HITEC regulations, and state law for communications and storage of electronic records, including encrypted email and teletherapy platforms. Please note however phone conversations and regular email, especially cell phones, are not ensured to be private due to the type of technology it is. I do not recommend text messaging except in unusual circumstances, and then only sharing of non-clinical information, i.e. appointment times. If you choose to call, text, or email me, you thereby consent to assume whatever risk is inherent in that form of technology. If you have questions, please let me know.

Financial Information:

My current outpatient therapy and consultation rate is $150.00 per 50 minute session.

Dr. Lane provides services through either private pay (Standard $150 per session) or certain third party (insurance) panels. For those preferring or needing private pay therapy services, Dr. Lane accepts cash, checks, and credit/debit cards (Visa, MC, AMEX, and Discover) which are due at the beginning of each session. Paying by credit card is typically the easiest way to pay for sessions, and you may keep a credit card number on file (secure) for convenience. In some cases you may pay for multiple sessions in advance, if that benefits you. Regarding returned checks, a normal fee of $35.00 is assessed for each check that is not honored by the bank. All payment balances must be up to date. Please don’t hesitate to discuss any and all issues or concerns around billing/payment with Dr. Lane.

When I contract with an insurance company to be part of their provider panel, I agree to certain terms, including a negotiated fee, that is usually less than my normal rate. This is a discount for those who have those insurance benefits and would apply to your deductible).

Sliding Fee Schedule: I do not have a set sliding fee scale. If a person is appropriate otherwise for my practice, I will try to work with them about ways to afford therapy.

Insurance: I accept the following: (As of 4/1/2020)

BC/BS of Texas (and most subsidiaries like Anthem, Magellan)

Aetna

Cigna

Meritain

Allegiance

There are many insurance plans within each provider, you need to learn what your specific plan will cover, your deductible and copay/coinsurance amounts, as well as any other limitations behavioral health care. Please call with your insurance information and we will see if your plans benefits will cover my services. Clients are responsible for any copay or deductibles for services, or if insurance denies a claim.

Before you start therapy with me, it is important that you become fully informed about your companies/plan benefits and reimbursement policies. Insurance reimbursement for my services is strictly between you and your insurance company.

Other insurance companies: If I am not a provider for your insurance company, I will be considered an “out-of-network provider”, so I have no contract with them to provide services. In this case, payment for services will be private pay. Do note, however, many companies will reimburse enrollees for a significant portion of necessary therapy costs for out-of-network providers, so it may pay to see what your company’s benefits are for out-of-network providers. I will provide to clients a bill/receipt that provides the necessary information to help them work toward their insurance plan reimbursement, if covered.

*Insurance will not pay for the No show or late cancellation fees, if incurred. This is your responsibility.
** Any legal related service fees will require 75% advanced payment of the estimated fee, reimbursable for time not spent.
